If you would like to schedule an appointment, please contact the office at (530) 229-1844.
Please give at least 24 hours notice to cancel or reschedule appointments to avoid a $50 missed appointment fee.
New Patients
We are only accepting new patients based on referrals from another physician. New patients should arrive 30 minutes early. Please download and complete the forms located here. Please bring your current medications, insurance information, driver license, any previous x-rays, medical records, and test results when you arrive for your scheduled appointment.
Patients should be aware that it may take up to two weeks for our office to receive any records from referring physician offices.
